What is the Care Coordination for Children Program?

This voluntary program helps families just like you find and use community services that are designed to support families. There is no change for your family to participate. If you have children from birth to age five who qualify for the program, your child service coordinator will work with you in meeting your family's needs. We can help with finding medical care, transportation, child care and/or financial aid. Your child service coordinator understands the importance of strong families and he or she be a source of information and support to you.

What are the goals of the program?

• To get you in touch with services for children and families.
  
  
• To support your children in reaching their developmental potential.
  
  
• To help ensure that children are raised in healthy, safe, and nuturing homes.

How can the Care Coordination for Children Program support my child and me?

Your Care Coordination for Children coordinator has information about a wide variety of family-oriented resources. He or she will:

• discuss family strengths and concerns through home visits, telephone calls and other personal contracts;
  
  
• identify programs, services and resources that meet your family's needs;
  
  
• provide developmental screenings;
  
  
• serve as a link between you and your child's doctor or nurse;
  
  
• suggest ways you can strengthen parent-child relationships;
  
  
• introduce you to parent support programs when available; and
  
  
• offer you encouragement and support.

Am I eligible for the Care Coordination for Children Program?

All children birth to five years of age with special needs are eligible. Other children in this age range may be eligible if there are significant concerns about their health, growth or development.



How much will the program cost?

Your family can participate in this program free of charge.



How can my family get a Care Coordination for Children coordinator?

You can call Martin-Tyrrell-Washington District Health Department at 252-793-3023 or you may be referred to this program through a variety of agencies and individuals. These may include family members, hospitals, doctors, nurses, early intervention programs, sickle cell disease programs, child care centers and others.
